The first recorded facial transplant was performed in France in 2005 on a 38-year-old woman whose nose, lips and chin had been bitten off by a dog. Tissues, muscles, arteries and veins were taken from a brain-dead donor and successfully transplanted to the patient’s lower face.
There have been only two similar attempts since. In 2004, the British Royal College of Surgeons published a controversial report predicting a high incidence of immunological complications for facial transplants. This data became a benchmark for facial transplant teams and review boards and greatly influenced the facial transplantation debate.
According to Steve Woodle, MD, of the University of Cincinnati, however, the report did not provide the best risk assessment. Both the health status of solid organ transplant recipients and the tissue composition of the solid organs reported in the earlier studies are very different from that of face transplant recipients and their facial tissues, he says. In the current study, the researchers compared the 2004 data with that taken from clinical studies describing kidney and hand transplants using the latest immunosuppression techniques.
They found the outcomes to be very different from those in the 2004 study. Based on kidney and hand transplantation cases, in which the same drug regimen was used, researchers found that acute, or immediate, rejection may occur in ten to 70 percent of patients. Of all these cases, however, rejection was reversible by adjusting the immunosuppression medications.
They also found that fewer than ten percent of patients would experience chronic rejection over five years. “In considering the most recent and relevant data, we came to the conclusion that the expectations for face transplant recipients should be significantly better than those previously published,” Woodle says.

Urban Smog Tough on Young Adults' Hearts
(HealthDay News) -- Dirty urban air drags down a number of "biological indicators" pointing to heart risk in young adults, a Taiwanese study shows.
The indicators include autonomic dysfunction, inflammation, oxidative stress, and blood coagulation, according to the study, which was published Aug. 15 in the American Journal of Respiratory and Critical Care Medicine.
"This study provides evidence that urban air pollution is associated with systemic inflammation/oxidative stress, impairment of the fibrinogenic system, activation of blood coagulation and alterations in the autonomic nervous system in young, healthy humans," wrote study lead author Chang-Chuan Chan of the National Taiwan University's College of Public Health.
Chan's team collected blood samples and performed electrocardiograms on 76 healthy college students once a month for three months. They compared the results to data from an air pollution monitoring station located on the students' campus.
They found that increased exposure to common air pollutants was associated with a significant rise in all biological markers of cardiovascular risk.
The study did not look at the precise biological mechanisms involved in this association.
"Further studies with more detailed measurements of cardiovascular endpoints over time are still needed," Chan noted.
"Most pollution literature has shown effects in elderly people, and although there have been experiments in young subjects, epidemiological research has not found such significant effects in young people as with these students in Taiwan," Dr. Benoit Nemery, professor in the division of pneumonology at the Catholic University of Leuvin in Belgium, said in a prepared statement. Nemery was not involved in the study.

Healthy Diet Guards Against Return of Colon Cancer
(HealthDay News) -- Colon cancer patients who eat a diet rich in fruits, vegetables, poultry and fish can significantly lower the risk of their cancer returning, new research suggests.
"We know a lot about how certain dietary things affect the risk of developing colon cancer in the first place but we didn't know, before this study, how diet affected persons who already have cancer," explained study author Dr. Jeffrey A. Meyerhardt, an assistant professor of medicine at the Dana-Farber Cancer Institute in Boston.
Although the findings, which appear in the Aug. 15 issue of the Journal of the American Medical Association, need confirmation, colon cancer patients might want to consider improving their eating habits.
"This is not a substitute for standard therapy, but it's not unreasonable for oncologists to use this data to start talking about diet," Meyerhardt said. "There are benefits in other regards, such as benefits for heart disease, and it does give us some initial information that may affect people's outcome."
"Maybe the message is it's never too late to change your diet," added Dr. Andrejs Avots-Avotins, an associate professor of internal medicine at Texas A&M Health Science Center College of Medicine and a gastroenterologist with Scott & White Hospital in Temple, Texas. "A healthy diet is going to be so helpful in so many different ways that even if you do end up with a cancer that may or may not have been related to your diet, this may be of benefit in prolonging your survival."
Diet and other lifestyle factors have been strongly implicated in the risk of developing colon cancer. It's been less clear what effect diet has on the course of established colon cancer.
The authors asked 1,009 patients with stage III colon cancer (cancer that had spread to the lymph nodes) who were participating in a chemotherapy trial to answer dietary questionnaires during and after the time they received treatment. All participants had already undergone surgery to remove the cancer.
The study was funded partially by the U.S. National Cancer Institute and Pfizer Oncology.
Two major dietary patterns were identified: Western (high intakes of meat, fat, refined grains and dessert) and prudent (lots of fruits and vegetables, poultry and fish).
A Western diet was associated with a significantly worst prognosis, both in terms of recurrence and death, than a prudent diet.
Compared with patients ranked in the lowest 20 percent of a Western dietary pattern, those in the highest 20 percent had almost three-and-a-half times the risk of recurrence or death. Those in the highest 20 percent of a Western diet were also 2.9 times more likely to see their cancer recur than those in the lower 20 percent.
"There's a biological basis for this. The Western type of diet affects insulin levels and insulin-like growth factors that help promote cancer's growth and metastases," Meyerhardt explained. "The magnitude of the effect was surprising, however."

Last Flu Season Was Mild, But Child Deaths Worrying
(HealthDay News) -- Although the 2006-07 flu season was comparatively mild in the United States, it still claimed the lives of 68 children, and experts say more must be done to reduce the death toll.
They're especially concerned about the steady rise in a potentially lethal combination of infection with flu and drug-resistant staphylococcus, or "staph."
"While waiting to see what this year will bring, we should all plan to roll our sleeves up and get vaccinated and in no way let our guard down," said Dr. David Katz, director of Yale University School of Medicine's Prevention Research Center.
In a report released Thursday by the U.S. Centers for Disease Control and Prevention, experts said the 2006-07 flu season peaked in mid-February. And there were actually fewer pediatric deaths and fewer children hospitalized with influenza than in the past three flu seasons, according to the CDC.
But, among the 68 children whose deaths were associated with flu from Oct. 1, 2006, to May 19, 2007, 21 had influenza plus Stapholococcus aureus ("staph") that was resistant to a leading antibiotic, methicillin, according to the report in the Aug. 10 issue of the CDC's Morbidity and Mortality Weekly Report.
In comparison, only one child died from flu coupled with S. aureus infection in 2004 through 2005, and three died during the 2005-06 season, the CDC said.
"We are looking into the number of deaths of children combined with staff bacterial infection," said Dr. Joe Bresee, CDC's chief of the Epidemiology and Prevention Branch in the Influenza Division. "From our perspective, it looks like it has increased."
Bresee said the CDC isn't sure if the number of deaths is actually rising, or the increase is due to better reporting. "The actual risk to kids from this is not known, but it is concerning," he said.
Bresee thinks that increasing the number of young children who get flu shots is key to reducing the problem. "They would be less likely to get influenza and, therefore, less likely to get co-infections with S. aureus," he said.
Despite the troubling news that such dual infections may be on the rise, the past season's flu statistics were relatively positive, Katz said.
"Data from the last flu season provide mostly good news," he said. "The number of cases was never unusually high. The strains were mostly of the expected varieties, and vaccine composition was just right. The death toll from complications of influenza was a bit lower than average."
The reasons for the milder flu season aren't clear, said Dr. Marc Siegel, an associate professor of medicine at New York University School of Medicine. "It could be increased surveillance and improved care and more awareness of what influenza can do," he said. "It may also be a less virulent strain of the virus."
Katz noted that, even in a good year, the average number of deaths from flu in the United States totals about 30,000, mostly among the elderly.
During the 2006-07 flu season, the influenza A (H1) virus was most common in the United States, but the A (H3) virus was found to be more common late in the season, according to the CDC.
For the 2007-08 vaccine, the U.S. Food and Drug Administration's Vaccines and Related Biological Products Advisory Committee has recommended that the trivalent flu vaccine contain A/Solomon Islands/3/2006-like (H1N1), A/Wisconsin/67/2005-like (H3N2), and B/Malaysia/2506/2004-like viruses, the CDC reported.
The influenza A (H3N2) and influenza B components remain the same for the new vaccine. These recommendations were based on antigenic analyses of recently isolated influenza viruses, epidemiologic data, vaccination studies in humans, and the availability of vaccine strains and reagents, the CDC said.
Last season, around the world, influenza A (H1), A (H3), and B viruses were most common. In Africa, small amounts of influenza A and B were seen. In Europe and Asia, influenza A (H3) virus was most common, but low levels of A (H1) were seen. Influenza B viruses were seen at lower levels in Asia and Europe but were most common in some countries, according to the CDC.
The best way to protect yourself from flu is to get vaccinated, experts said. "We had a good flu year. But that was last year," Katz noted.
In terms of the H5N1 bird flu virus, from December 2003 to mid-July 2007, 319 cases of bird flu in humans were reported to the World Health Organization. Among these cases, 60 percent (192) were fatal. To date, no human cases of bird flu have been reported in the United States.
All human cases were reported from Asia (Azerbaijan, Cambodia, China, Indonesia, Iraq, Laos, Thailand, Turkey, and Vietnam) and Africa (Djibouti, Egypt, and Nigeria).
But, "the threat of an avian flu pandemic still looms," Katz said. "To date, deaths from avian flu have been few, and limited to Asia. But no one knows for sure when that might change."
Siegel, author of Bird Flu: Everything You Need to Know About the Next Pandemic, noted that although H5N1 has a high mortality rate, "we see year after year that it is confined to birds.
There is no indication that it is about to become a massive human problem. There is no indication that it is on the verge of transforming."

A Powerful Approach to Prostate Health
Most men hardly know they have a prostate until they reach middle age, when prostate health becomes a serious issue. Prostate enlargement is found in 50 percent of American men in their sixties, and up to 90 percent of men in their seventies and eighties. Worse, prostate cancer is the second leading cause of death by cancer in men, with about 80 percent of all cases of prostate cancer occurring in men over sixty-five.
Even though the prostate seems to be an older man's health problem, these imbalances begin much earlier in life. The good news is that most prostate problems can be prevented by making simple lifestyle and dietary changes to restore balance. In this interview, our ayurvedic expert provides an in-depth analysis of prostate health from the perspective of Maharishi Ayurveda, and gives practical and effective advice for keeping the prostate healthy throughout life.
Q: Let's start at the beginning. What exactly is the prostate gland, and what does it do?
A: The prostate is part of the male reproductive system. The interesting thing is that even though it is called a gland, it is actually an organ made up of 70 percent glandular and thirty percent fibro-muscular tissue. About the size and shape of a walnut, it is located directly beneath the bladder and surrounds part of the urethra, the tube that drains the bladder. The prostate secretes a thin, milky substance contained in semen that protects and nourishes the sperm and lubricates the urethra. In ayurvedic terms, the prostate's secretion supports the flow of Shukra Dhatu, the reproductive tissue.
Q: How would you know if you had a prostate problem?
A: One major symptom is prostate enlargement, which is caused by imbalances that accumulate over time but become manifest with age. The initial symptoms include excessive urination at night, a weak urine stream, stopping and starting while urinating, a feeling that the bladder is never empty, difficulty in starting to urinate, and dribbling at the end of urination.
If a man has these symptoms, he should consult his doctor immediately. Often, when the symptoms are mild, the physician will say, "Wait and watch. This is the ideal time to get help from Maharishi Ayurveda, when the problem is mild to moderate. At this point, there is still an opportunity to correct the imbalance that has caused the problem. Of course aging is the main etiological factor, or cause of the problem, but Maharishi Ayurveda also identifies the lifestyle or dietary factors that act as a trigger, and if unchecked, can turn a mild problem into a serious one.
Q: Every man would naturally want to keep the problem from becoming serious. What are some of the causes, or triggers, of prostate problems?
A: There are several. One is overuse of Shukra Dhatu, or overuse of the sexual organs. Another is lack of adequate fluids in the body, due to not drinking enough water during the day. Habitually suppressing the natural urge to urinate also can, over time, cause the urine to become more concentrated and, in turn, irritates the prostate.
Drinking too much alcohol or caffeinated beverages can also cause an imbalance. Alcohol, for instance, creates an abnormal increase in urine production, which aggravates Pitta and Vata doshas. Another cause is taking too many over-the-counter antihistamines and decongestants. These can cause the muscle that controls urine flow to tighten, making urination difficult.
Even your job can be a source of imbalance. If you have to sit for long periods of time without taking a break or stretching, this can create an imbalance in the reproductive area of the body. Not getting daily exercise, or eating a diet that creates impurities (ama) in the body, are other causes of prostate problem. If your diet does not contain spices that purify the urine daily (such as turmeric, cumin and fennel) that can also cause toxins to build up that lead to imbalances or infection.
Finally, if you don't have a healthy daily routine, for instance if you watch TV late at night or for other reasons don't sleep enough, or if you don't eat meals on time or your routine is very irregular, that can also cause ama to build up and disturb the immune system.
Q: It sounds like a wide number of factors can cause prostate problems.
A: Yes, they do sound like they are widely disparate on the surface, but actually there is an underlying principle that ties them all together.
One main factor is ama. Ama refers to the sticky impurities that are created when digestion is weak and food is not digested completely. Now, aging itself can create some weakening of the digestion, but most ama is created by poor dietary and lifestyle habits already mentioned -- eating foods that are old and heavy or eating meals at irregular times of the day.
If this kind of diet becomes a habit, then ama eventually mixes with the nutrient fluid (Rasa Dhatu) and even the blood (Rakta Dhatu). The urine, which is part of the body's purification system, also becomes overloaded with ama and thus more prone to infection. It becomes a fertile ground for bacteria, and can create additional problems of urinary tract infections, and that impacts the prostate. Weakened immunity also creates a problem for the prostate.
If the ama problem is not corrected, and ama reaches the muscle and fat tissue, then it creates an additional factor -- imbalance in cell production. This, compounded with testosterone levels declining due to age (or due to overuse of the sexual organs), is a major factor in prostate enlargement.
If the person also has an imbalance in Shleshaka Kapha (the subdosha of Kapha that governs lubrication of the joints, body fluids, and moisture balance in the skin), and ama is accumulating in the body fluids due to dietary mistakes, then Shleshaka Kapha and ama gets mixed with urine, creating a more irritated situation and further weakening the immune system.
Also, when Shleshaka Kapha is mixed with ama, it creates shleshma (a sticky, toxic mixture of Shleshaka Kapha and ama ). This condition is a common factor in imbalanced cell production. When combined with the problems of ama spreading to the tissues mentioned earlier, then the cell production becomes abnormal, and the prostate grows abnormally large.
Maharishi Ayurveda tries to support health on all fronts: by balancing Apana Vata, the subdosha of Vata related to the downward flow of energy such as occurs with urination, stopping ama from being produced, strengthening the immune system, and cleansing the urine and nutrient fluid, and cleansing the blood, muscle and fat tissues of ama, and cleansing Shleshaka Kapha of amavisha.

These include canned, frozen, packaged and processed foods, as well as leftovers. Buy your fruits and vegetables fresh, and if possible, buy organic foods. Non-organic foods contain pesticide and chemical residues that can irritate the blood tissue and pollute the body with toxins. This, in turn, overloads the elimination systems such as the urine and irritates the prostate. Warm, cooked, light, and mildly spiced foods, on the other hand, are easy to digest and thus help cleanse the body of ama.
Q: What about lifestyle, doesn't that affect the doshas as well?
A: Absolutely. Here, it's the same principle: avoid extremes in lifestyle and routine. Moderate habits and daily routine help remove ama and pacify all three doshas.
Lifestyle habits that aggravate Vata dosha include eating at different times of the day from one day to the next. You'll want to eat your meals at the same time every day, and eat your largest meal at noon, when the sun is at its zenith, as that is when your digestive powers are also the strongest. Take time to eat nourishing, appetizing meals. Avoid staying up late (past ten o'clock), as night wakefulness aggravates all the doshas, but especially Vata. Go to bed at the same time every night, before ten o'clock, and rise before six o'clock in the morning. Sleep is necessary to pacify Apana Vata and all aspects of Vata. It is also important in enhancing the immune hormones, eliminates ama and creates more ojas, the product of good digestion that promotes lightness, bliss, immunity, health, and longevity.
Be sure to exercise regularly, and be sure that the exercise you choose is suitable for your age and body type. Daily ayurvedic massage, called abhyanga, is another important part of the daily routine. You can do it in the morning to help improve circulation and digestion, remove ama from the body, tone the muscles, and increase energy.
Avoid too much mental pressure, such as a job that requires you to work long hours. Take time to enjoy life, and to maintain healthy relationships with your family and friends. Practice a technique to reduce stress, such as the Transcendental Meditation® technique. A: Yes, here are some tips, which also summarizes what we just talked about:
Drink lots of water throughout the day, in between meals. This will keep the urine from becoming too concentrated. If you add some herbs to the water, it will flush out toxins, dissolve ama in the blood tissue and help purify the urine. Here's a recipe: Boil two quarts of water and put the water in a thermos flask. Add three leaves of holy basil, 1/3 t. fennel seed, 1/4 t. coriander seed, and six white pumpkin seeds or six fresh cucumber seeds. Keep drinking this water throughout the day, but stop drinking it after 7:00 p.m. Stay away from fluids at night if you have a prostate problem, as a full bladder at night can cause pressure on the prostate and disturb your sleep with the need to get up frequently to urinate.
Each time you urinate, take the necessary time to empty your bladder completely. Because the urethra (the passageway through which the urine drains from the body) is already narrow, when there is some enlargement of the prostate, the urinary channel becomes even narrower. If you are in too much of a hurry, all of the urine might not be drained from the bladder. This is not a good thing, because the urine becomes more concentrated if it is not drained from the bladder each time you urinate. So take the time to empty the bladder completely.
Avoid drinking alcohol, because it creates imbalances in the blood tissue, as does caffeine. Both of these create abnormal urine production and irritate the bladder as well.
If you have to sit all day at your job, take short breaks frequently and try to stretch or take a short walk. This will restore the normal flow of energy, blood, and nutrient fluid to the prostate area.
When the weather is cold, take care to keep your head, neck, and body warm. If your body temperature drops, it leads to urine retention, which aggravates the prostate. So keep your body temperature in a moderate range.
Make sure that your bowel movements are regular. Constipation creates an obstruction in Apana Vata, in turn creating pressure on the prostate, increasing ama in the blood tissues, and creating excessive dryness in the whole area. Doctors 'Awaken' Man 6 Years After Severe Brain Injury
(HealthDay News) -- In a scene reminiscent of Oliver Sacks' book Awakenings, doctors have managed to partially rekindle the mind of a man who had been in a minimally conscious state for six years.
The 38-year-old patient, who was severely brain-damaged, received deep brain stimulation and can now feed himself and communicate, activities that were unthinkable before the therapy.
This is the first time that deep brain stimulation has been used on a person with a traumatic brain injury in a minimally conscious state, and it offers some hope for others in similar situations, the researchers said.
"Hopefully, this will now begin to open doors that were closed up to this point," said Joseph Giacino, study co-investigator and associate director of neuropsychology at JFK Johnson Rehabilitation Institute and at the New Jersey Neuroscience Institute in Edison, N.J. "There's a very nihilistic view that when a brain is badly damaged there's not much we can do to change that. We have a very tough time getting research funded because of the need to jump higher than most other research. By demonstrating that we did move the bar in this patient this late, we hope that it's going to force people to revisit this somewhat nihilistic view."
Lead study author Dr. Nicholas Schiff, an associate professor of neurology and neuroscience at Weill Cornell Medical College in New York City, said: "We set out with a goal to try to identify a set of patients who might, if this was successful, regain functional communication and we thought that was important because being able to communicate is important, even if you remain disabled, because you can reengage with people in your immediate environment, including family and the people taking care of you and give a better idea of your needs. We were able to do this for this patient. Whether this means we'll be able to do this again, we don't know until we do it again but we do think there will be other patients who can respond to this. It's a first step to building a science to do this."
The patient was the first of 12 patients to try deep brain stimulation in a U.S. Food and Drug Administration-approved trial.
Dr. Ali Rezai, senior author of the paper and director of the Cleveland Clinic's Center for Neurological Restoration, said, "There's a great period of scientific discovery coming with respect to traumatic brain injury. This had been a great big desert of unknowns. There are 1.5 million new cases of TBI [traumatic brain injury] in the U.S. each year and two-thirds of Iraq war veterans have TBI. This is an exciting time to come."
The finding is published in the Aug. 2 issue of Nature.
People in a minimally conscious state can show sporadic evidence that they are aware of themselves or their environment. The state is not the same as a persistent vegetative state or a coma.
An estimated 100,000 to 300,000 traumatic brain injury patients in the United States are currently diagnosed with minimally conscious state. Most do not receive active rehabilitation and are relegated to long-term facilities.
"These patients are devastated and their families are devastated. They are taken to nursing homes and chronic-care facilities and forgotten about," Rezai said.
But some patients in a minimally conscious state, including the man in the new study, do retain brain activity. In one other recent high-profile case, 39-year-old Terry Wallis of Mountain View, Ark. regained speech and movement two decades after a truck accident left him in a minimally conscious state. Some of the same researchers involved with the new study also documented the Wallis "awakening," proposing that the re-growth of neural networks was responsible.
The patient who is the subject of the new study underwent a 10-hour operation during which electrodes were implanted into his brain and used to stimulate the thalamus on both sides of the brain. The procedure has been used before in Parkinson's disease, dystonia, Obsessive-compulsive disorder and depression.
According to Rezai, the procedure required much more precision when used for minimally conscious state. "You have to target specific parts of the brain with millimeter precision using various computer-generated brain maps and physiological mapping," he explained.
The patient also received other forms of rehabilitation.
Within 48 hours, the patient turned his head in response to voices and kept his eyes open for prolonged periods of time. After more time, the patient, who previously could not talk, started naming objects and using objects, such as a hair brush, with his hands.
After an initial "titration" period during which the dosage and timing of the stimulation were fine-tuned, the patient began a six-month period during which periods of stimulation were alternated with periods of no stimulation.
Communication, motor control and feeding were the most improved when the brain was receiving electrical impulses, but were still enhanced even when the device was turned off.
"What we did here was facilitate and augment neural networks that were already in place," said Dr. Joseph J. Fins, study co-investigator and a professor of medicine and public health and chief of the division of medical ethics at Weill Cornell Medical College. "This is distinct from permanently vegetative patients like Terri Schiavo. You need an intact set of circuits to work with and he did have an intact set, except he was not able to sustain their activation, as it were."
Giacino added: "We were able to influence this patient's course of recovery very late in the process. No treatment has been proven to accelerate the course of recovery from brain injury or improve outcome at the end of day. At least in one patient, we think we've convincingly shown this is possible."
There are still many unknowns, however, including whether this patient will continue to improve and whether deep brain stimulation will help other brain-injured patients.
"This patient continues to improve outside of the trial with the stimulator in place," Fins said.
"But we're in uncharted territory about what kind of degree of recovery he might ultimately achieve."
"It's very dicey stuff, brain injury, because the next patient we enroll in the study, that brain injury will not be the same because no two are alike," Giacino said. "We need to continue to replicate the findings and figure out who would benefit from this."
Dr. Jonathan Friedman, director of the Texas Brain and Spine Institute, agreed. "This could have huge implications but it's all speculative with just one patient," he said. "Every head injury is very different. It's probably naive to think we could stimulate every patient in the same way, although some might benefit even more."
